Product Certification Application

An Application Form for each product to be certified must be submitted to Guardian.

If the product is part of a series, all model numbers to be certified must be indicated. This form may also be used for any changes/updates needed to original Product Certifications.

A $200 non-refundable Application Fee is required in order to begin the certification process.
